ECHS POLICY FOR CHANGE OF
POLYCLINIC
1. As
per policy issued vide Central Org ECHS No B/49711/AG/ECHS 25
Mar 2011, it is responsibility of New ECHS polyclinic to forward an intimation
regarding outstation member joining the Polyclinic as permanent members (opting
for parent Polyclinic) to previous (old) parent Polyclinic and RC as per format
att at Appx ‘A’ to this policy letter.
2. Some
ECHS Polyclinics are pressurise veterans to bring Appx A duly signed from
previous (old) parent polyclinic by hand resulted into heavy expenditure to
members specially outstate/district. They reassign their responsibility to
members by violating the Para 5 (a) to (d) of policy.
3. If any violation on policy by
ECHS Polyclinic for their convenient,
the matter may please be reported to OIC ECHS/Station HQ.

Very most useful thaks for info one thing can you intimate ple i applied for new echs smart card but i have not got since two yeras i have only slip

DeleteWhat is the process of new echs cards and change of address at cards?
ReplyDeleteApply for new card only after execution of new contract for prep of smart card. Procedure exist for the same.
